Reporting Symptoms and Exposures
If you are notified of or witness someone with COVID-19 symptoms (ex. fever, cough, difficulty breathing, loss of taste or smell, etc.) please encourage them to:
- Stay Home
- Contact a healthcare provider and report symptoms and any pre-existing conditions
- Practice socially distancing
- Complete the COVID-19 Reporting Form
